Williams Hematology ((LINK))

William J. Williams, MD, left a unique stamp on the field of hematology through his exemplary patient care and research. He also wrote "the book" on the field, editing what became for most of the 1970s and 80s one of the two most familiar English-language hematology textbooks. The book, originally called Hematology, remains in wide use along with an expanding group of other excellent texts. To honor his contributions as founding editor, the name of the book was changed to Williams Hematology beginning with the fifth edition.

Each fellow has a primary site Clinic where they follow a diverse cohort of patients throughout their fellowship training with a dedicated Clinic Attending who also serves as a mentor. During the entire fellowship they have a half-day once-weekly session and they have exposure to benign and malignant hematology as well as mix of solid tumor oncology cases. Fellows also participate in multidisciplinary care of their patients alongside Surgical Oncology and Radiation Oncology physicians. Fellows have the opportunity to perform oncologic procedures (bone marrow aspiration & biopsy, intrathecal chemotherapy) and develop proficiency in writing chemotherapy orders for their patients.

When he joined the faculty of the State University of New YorkUpstate Medical Center in 1969, he became the Edward C. ReifensteinProfessor of Medicine and Chairman of the Department of Medicine, wherehe served for 22 years. In his early years, he attracted excellentfaculty members including Arlan Gottlieb, Robert Comis, and KennethZamkoff, who were the leaders of hematology-oncology at that time, andtheir legacy continues to this day.

Long revered for its comprehensiveness and extraordinary depth of detail, Williams Hematology provides essential coverage of the origins, pathophysiological mechanisms, and management of benign and malignant disorders of blood and marrow cells and coagulation proteins. The text contains a wealth of basic science and translational pathophysiology for optimal, lifelong learning. Experts in research and clinical hematology, the editors are known worldwide for their contributions to the field.

Josef T. Prchal, MD, earned his medical degree from Charles University in Prague, Czech Republic, completed his residency in internal medicine at Toronto General Hospital and his hematology fellowship at Toronto General and Sunnybrook Hospital, and served as a research fellow at the City of Hope Medical Center in Duarte, California.

Reviewer: George M Rodgers, MD, PhD (University of Utah School of Medicine)Description: This is a comprehensive textbook that attempts to summarize all the major clinical and foundational concepts of pathophysiology related to the fields of benign and malignant hematology. As with previous editions, it is divided into over 100 chapters organized into 13 general sections, starting with chapters on fundamental clinical and biological topics, including clinical and pathological evaluation of hematology patients, normal blood development, basic concepts in molecular biology, genetics, and immunology, and general principles of therapeutic agents used in the treatment of hematology patients. Following these "foundational sections" are the more clinically focused sections on the various benign and malignant blood disorders, organized into sections for each major blood cell type (benign versus malignant): erythrocytes, myeloid cells, lymphoid cells, and platelets and coagulation. Each of these sections begins with one or more general chapters on the normal structure and function of each cell type as well as a chapter on the classification of disorders related to each cell type before getting to the chapters addressing individual blood disorders or groups of related disorders. Finally, there is a section on transfusion medicine. Each chapter is written by one or more authors who are universally recognized authorities on the topics about which they are writing. Each chapter is organized in the same manner beginning with a brief summary and concluding with an extensive and up-to-date list of references; otherwise, the chapters are divided as appropriate into various sections and subsections supported by high-quality illustrations, clinical photographs, tables, charts, and other figures to support the text. The previous ninth edition was published in late 2015.Purpose: In the preface, the editors state that the purpose of this edition, as with all previous editions, is to "provide the most current concepts of the genetic basis, pathophysiology, diagnosis, and treatment of hematologic diseases." They also note that a new edition was needed because of recent advances in hematology, particularly as a result of new technologies in genetics, genomics, and proteomics, which have led to new advances and emerging therapies such as gene therapy and immune therapies. They also emphasize the importance of blood cell morphology and pathology in the diagnosis of hematologic disorders, and thus a major goal of this work is to provide a comprehensive visual reference. Finally, they emphasize the continued contributions of renowned experts in their particular fields within hematology in writing the individual chapters to ensure their completeness, accuracy, and currency. These objectives are quite worthy, as hematology, like every medical subspecialty, needs its comprehensive textbooks. As it has been more than five years since the most recent edition of this work, a new edition was due, particularly given all the advances during that time, including immune therapies such as CAR-T cells and new emerging entities such as CCUS and CHIP. This book meets its objectives as it provides an excellent comprehensive resource on all aspects of hematology, including foundational concepts as well as individual disorders with their pathophysiologic descriptions, diagnostic criteria, and treatment. The chapters are complete, up to date, and expertly and clearly written with excellent references and microphotographs, diagrams, and other figures.Audience: The intended audience is practicing clinicians, particularly hematologists and hematopathologists, and trainees in those areas, including medical students, residents, and fellows. Additionally, I think other "generalist" practitioners, including adult and pediatric primary care practitioners and hospitalists, might also find many parts of it useful for their day-to-day practice, as might clinicians in a wide spectrum of other specialties and subspecialties. Laboratory-based, non-clinician translational researchers might also find benefit in consulting this work to enhance their understanding of the clinical context of their research questions. In general, it is written in such a way that it is accessible to almost anyone with advanced training in the biomedical sciences. With its clear text, helpful figures, and extensive background and references for further reading, I think almost every reader will find this book to be a useful resource for investigating any issues related to hematology and blood disorders they might encounter in the course of patient care or their research endeavors. Most readers probably would not read such a large book "cover to cover," but instead use it as a reference book to answer particular questions. To that end, there is a very extensive index, and the sections and chapters are logically organized and clearly titled so that readers should be able to easily navigate this large work without it becoming overwhelming. The editors of this book and the contributing authors of the individual chapters are all internationally well-known experts in the fields about which they are writing with numerous high-impact publications and reviews on these topics.Features: There are many features that make this book exceptional, besides its comprehensiveness. It is clearly written, and each chapter is fairly complete with cross-references to other chapters as needed and extensive and up-to-date endnote references at the end of each chapter. The photomicrographs of blood smears and pathology specimens are superb as are the other figures and diagrams, which greatly aid in clearly describing the content to the reader. A textbook of this size could easily be overwhelming, but the sections and chapters are well organized and the index at the end is very comprehensive enabling the readers to easily find what they are looking for. While the malignant hematologic disorders are generally well covered, the book's greatest strength is its treatment of benign hematologic conditions, and its chapters on foundational concepts pertinent to the understanding of normal blood development and the pathophysiology of malignant and benign blood disorders, including the chapters on genetics, biochemistry, and cell biology as they pertain to hematology, as well as the several chapters on basic concepts of immunology, which are areas of rapid progress with numerous implications for the understanding of multiple hematologic disorders and their treatment, not least of which are the emerging immune therapies and CAR-T cells. These latter topics represent some of the major updates in this edition. There are really no glaring omissions in this very comprehensive book, and I think this work does a good job balancing depth and breadth with excellent references for readers wishing to pursue any topic more in depth in the primary literature.Assessment: This book, like its preceding editions, is an excellent resource for physicians diagnosing and treating the full spectrum of hematologic and coagulation disorders. It is expertly and clearly written with an appropriate amount of detail with abundant up-to-date references for readers who want to go more in depth in the primary literature. Hematology specialists will appreciate the updated content, particularly regarding the new immune therapies and CAR-T cell therapies that have emerged since the last edition, as well as updates on the new categories of clonal myeloid disorders such as CHIP and the advances in bioinformatics that are shaping contemporary and future hematology research. Additionally, non-specialist clinicians will find this to be a helpful resource in commonly encountered hematologic conditions such as the common anemic conditions and bleeding and clotting conditions, which are often diagnosed and managed by primary care providers and hospitalists. This work has become one of the standard textbooks in hematology, and given the many huge advances mentioned previously, a new edition was needed to build upon the most recent one that appeared about five years ago. Almost all fields of hematology have witnessed varying degrees of progress and thus, besides just adding chapters such as the one on CAR-T cells, updates on all the other chapters of the previous edition were needed as well. Again, one of the great strengths of this title is its treatment of benign hematologic conditions, ranging from the common nutrition-associated anemias, immune-mediated cytopenias, and disorders of thrombosis and hemostasis, to the less common genetic conditions including rare bone marrow failure syndromes and enzyme disorders. These conditions often receive less emphasis in hematology books compared to malignant diseases such as leukemias and lymphomas, but here they are given appropriate space. Comparing this work to its major competitor, Wintrobe's Clinical Hematology, 14th edition, Greer (Wolters Kluwer, 2019), it is generally hard to say that one is better than the other. Both feature expert authors for each of their chapters, which showcase clear and concise writing, excellent figures, and abundant and timely references. They each give appropriate space to malignant and benign hematologic conditions, but they have slightly different organization of the content. However, since either of these titles would be used a reference resource, the exact order of the individual chapters seems not to matter very much, as both are easily navigated by the table of contents or index. In both cases, online editions are available for easier use given how large and heavy these books can be. Most readers will probably access this content online via institutional subscriptions, and perusal of the online versions of both titles indicates that both are easy to use.
